In order to be seen by our staff.... please bring a complete updated list of any medications you are taking as well as the name and phone number of the doctor(s) prescribing them, your insurance cards and subscriber information and if you would like, you may print our New Patient Forms to bring with you. 

What else should I know?

If it’s your first trip to Harrah Family Dentistry, please plan to arrive 10-15 minutes early with all the necessary forms. This will ensure we can get you in at your scheduled appointment time.
